If you have a serious illness and are in agreement with the goal of comfort care, we are here to help you. We serve people of all ages. Community Hospice provides an enhanced quality of life for people with illnesses that include:

Community Hospice's highly skilled comfort care team provides services in a patient's home, local community hospitals, skilled nursing facilities, assisted living facilities, group homes and other residential settings. We will assist patients and families in the care setting of their choice and when needed, we will ease the transition from one care setting to another.

Medicare, Medicaid and most private insurance providers cover all hospice care costs, including medications, equipment and supplies that relate to the terminal diagnosis.
Through community donations, we are able to care for all who need our services. No one is refused hospice care due to inability to pay.

Community Hospice is committed to offering a high quality of care to ensure that those entrusted to us receive the exceptional service. The following is a list of services and supplies provided to you:

When should I contact Community Hospice?
When the goal of treatment begins to shift from curing the illness to providing comfort, it is time to consider Hospice care. This may come well before a physician indicates that the patient's life expectancy is limited. Unfortunately, many people wait until their final days to involve Hospice care. By contacting Hospice early, the patient and family are informed of their options and may choose the path that will have the most positive impact on their life. The best time to learn about hospice is before you need it. We are available to meet with you and your family to help determine the right time for services to begin.
Back to topWhat happens if I show signs of recovery?
It can be hard for doctors to know how long a person will live. Some people live longer than expected. If you live longer than six months, you can continue on Hospice as long as you meet eligibility criteria.
There are times when a patient's medical condition improves and Hospice services may no longer be appropriate. At that time our hospice team of professionals will work with you to establish an alternative plan of care. If at a later time you are in need of Hospice services, Medicare, Medicaid and most private insurance will provide coverage for your return to our program, based on eligibility criteria.

it be used?
As a nonprofit Hospice we receive support and donations from the community. We respond to the needs of the community by creating programs and services, sharing our resources and spaces and partnering with local businesses and organizations. All of our donations are directly used for helping patients, their families and our community.
